Is Connected Health a full replacement for primary care?
Yes! Connected Health manages all primary care needs (and more!). We become the quarterback of your healthcare.
What is included in my concierge membership?
  • Comprehensive baseline visit and preventive wellness assessments
  • Initial onboarding physical exam
  • Personalized Care Plan
  • Annual physical exam
  • Quarterly wellness exams
  • Unlimited visits with your Care Team (in-person or virtually)
  • VIP medical appointments scheduling & coordination
  • Point-of-care tests
  • On-site lab work (labs billed to insurance)
  • Comprehensive lab reviews
  • Ordering of prescriptions & refills (medications are billed to insurance)
  • Flu vaccinations (all other vaccinations can be submitted to insurance for reimbursement)
  • Access to our preferred network of specialists
  • Medical records retrieval & management
  • Assistance with insurance, prior authorizations, and scheduling of all medical appointments—both in-office and with outside specialists
  • VIP access to aesthetic consults (Botox, filler), personal training, physical therapy, chiropractic care, and hyperbaric oxygen therapy
  • On-site pharmacy
What is not included in my membership?
While we are happy to coordinate the services below, their costs are not included in your membership:
  • Diagnostic imaging (MRIs, ultrasound, mammograms, etc.)
  • Specialist visits
  • Emergency room or urgent care visits
  • Hospitalizations
  • Cost of lab work, prescriptions, vaccinations (preferred on-site, but billed to insurance)
  • Optional or advanced testing
Who is part of my Care Team?

Your care is collaborative and truly connected.

  • Lead Physician (MD): Think of them as the quarterback of your health–leading your care team, coordinating your care plan and keeping your health goals on track.
  • Extended Care Team: Physician assistants, nurse practitioners, nurses, medical assistants, and administrative specialists working together on your behalf.
Do I need to carry health insurance with my membership?
Health insurance is a personal choice; however, our recommendation is that you continue to carry health insurance to cover lab work, vaccinations, prescriptions, imaging, diagnostic tests, and hospitalizations. Many members choose a higher-deductible, lower premium plan and invest more directly in primary care, where up to 80% of health needs can be addressed. HSA or FSA funds may be used toward membership fees; please check with your tax advisor or plan administrator for eligibility.
What happens if I need to go to the emergency room or be hospitalized?
If hospitalization is required, our physicians work closely with trusted hospitalists and specialists. Our team stays involved to help coordinate care and keep your experience as connected and seamless as possible.
How will I be billed?
Your monthly membership begins on the day of your baseline visit and is automatically billed each month thereafter.
What are the next steps to get started?
New members begin with:

Baseline Exam: (In-Person, 2 hours):
A comprehensive visit to review your medical history, concerns, and priorities. This includes a full physical exam, on-site lab work, and health assessments.

Care Plan Review (Virtual, 30 minutes)

Held 2-3 weeks later to review results, discuss your personalized Care Plan, and outline next steps. Ongoing check-ins help support your progress.

What if I need to cancel my membership?
You can cancel your membership at any time with 30 days’ written notice prior to your next billing cycle. Cancellation requests must be submitted via email or written letter.
What if I don't live in Pittsburgh?
Baseline exams and annual physicals must be completed in person. Many other services can be provided virtually, depending on location and care needs.
I have to cross a river to get to you?
Yes—and we believe your health is worth the drive. Our office is conveniently located in Wexford just off I-79, and with telemedicine, many visits can take place virtually from the comfort of your home.
