Is Connected Health a replacement for primary care?
Yes! Connected Health manages all primary care needs (and more!).
What is included in my concierge membership?
Comprehensive baseline visit and preventive wellness assessments
Personalized Care Plan
Unlimited communication and visits with your Care Team (in-person or virtually)
Point-of-care tests
On-site lab work (labs billed to insurance)
Ordering of prescriptions & refills (medications are billed to insurance)

What is not included in my membership?
We are happy to help coordinate the services listed below, however their costs are not covered by your membership.
Diagnostic imaging (i.e., MRIs, ultrasounds, mammograms)

Cost of lab work, prescriptions, vaccinations (done on-site, but billed to insurance)

Who is part of my Care Team?
Your practitioners and care team collaborate to ensure your care is "connected".
Lead Physician (MD): Baseline exam, annual physical, preventive care, and sick visits (if available)
Care Team: Made up of physicians, physician assistants, nurse practitioners, nurses, and administrative specialists.
Do I need to carry health insurance with my membership?
Health insurance is a personal choice; however, our recommendation is that you continue to carry health insurance to cover lab work, vaccinations, prescriptions, imaging, diagnostic tests, and hospitalizations. We'll often recommend our members move towards a higher deductible, lower premium plan to front-load their healthcare spend on primary care. Why? Because 80% of health needs can be addressed through primary care alone. HSA/FSA dollars may be used to cover all or some of your membership fees, but we encourage you to check with your tax advisor or HSA/FSA plan administrator to confirm.
What happens if I need to go to the ER or become hospitalized?
Our doctors work with a group of reliable hospital physicians who can be entrusted with your administrative care. Our medical team will work closely with the hospital staff and specialists to continue keeping your care as "connected" as possible.
How will I be billed?
Your monthly membership starts the day of your first baseline visit. You will be auto-charged monthly thereafter.
How do I take the next steps?
New members start with a two-hour baseline exam (in-person) and a 30-minute Care Plan Review (virtual) in the following weeks.

Baseline Exam: This is when you will tell us about your medical history, pain points, and health & wellness goals. This first visit is two-hours long, in person. During the exam you will have a full physical, lab work, and health assessments conducted on-site.

Care Plan Review: A follow-up with your care team will take place 2-3 weeks after your baseline exam. This virtual meeting will be used to review your lab work and discuss your Care Plan. Your team will check in regularly to help ensure you are making progress.

What happens if I need to cancel my membership?
You can cancel your membership any time with 30 days' notice before your next payment is due to stop future payments. A request for cancellation must be done by email or written letter.
What if I don't live in Pittsburgh?
Our baseline exam and annual physicals must be in-person, however most other services can be rendered virtually.
I have to cross a river to get to you?
Yes, but we believe your health is worth the drive! Our building is conveniently located in Wexford off I-79. And with telemedicine, many appointments can take place virtually from the comfort of your home.
