Connected Health does not participate with any insurance company. The membership fee is not covered by insurance. Individuals who want to be part of a smaller, more personalized service pay this fee directly out of pocket. We understand this is a different model than you are used to. However, data shows that comprehensive primary care saves patients money over the long term.
Health insurance coverage is, of course, your personal choice. We do believe having insurance is important. We encourage our patients to maintain other coverage for services that might be needed outside of our care, such as specialists and emergency room visits. Our service pairs perfectly with todays high-deductible plans. This is something we discuss in greater detail before your first physician visit.
Absolutely! But just like private insurance, our fee is not reimbursable under Medicare. Your Medicare coverage will remain the same for you as it has before.
Our doctors work with a group of reliable hospital physicians who can be entrusted with your admission and care. Our medical team will work closely with the hospital staff and specialists to continue quarterbacking your care during your stay, and after.
Your physician will still be available to you by phone any time he or she is away. When your personal physician is away, another doctor will be here to assist with any medical needs that come up. As a patient, you'll be notified in advance of your doctor's unavailability.
Yes! We have an optional program for house calls. Sick visits, exams, even blood draws can take place right in the comfort of your home or office.
Many of our services can be purchased à la carte; no membership required!
Yes! There are many ways we can work with companies to help meet the needs of your employees while helping your company meet its health savings goals. From testing, to telemedicine, many of our services can be offered to employers. We encourage you to call to discuss your specific company needs.
Yes, but we believe your health is worth the drive! Our building is conveniently located in Wexford off I-79.
