FAQ's

What should I bring to my appointment?
1. The I-693 Form provided by Connected Health with the appropriate information completed.
2. A record of your immunizations. This document must be translated into English.
3. COVID vaccination card
4. Photo ID such as a driver's license or passport.
Do you complete immigration medical exams for children?
Connected Health is happy to complete immigration physicals for patients age 8 and older.
How do I pay for my exam?
Connected Health gladly accepts cash payments or credit card.
Is there a fee to schedule an appointment?
A booking fee of $50 is required to schedule an appointment and goes toward the total cost of your exam. If you choose to cancel your appointment with more than 24 hours notice, the $50 booking fee can be refunded.
Can you bill my exam to my health insurance?
Connected Health does not submit to insurance. All costs associated with the immigration medical exam are due at the time of service.
I cannot find my immunization record. What do I do?
All vaccinations must be officially documented. If you are unable to provide documentation of past vaccines, you will need to be re-vaccinated. Some lab work can determine if certain vaccines can be omitted, such as varicella.
How do I take the next steps?
When you are ready to schedule your immigration medical exam, call our front desk at 724-933-4305.
Our team will collect your information, schedule your appointment, and apply your $50 booking fee.
You will receive an email containing the I-693 Form as well as a link to complete our office intake records.
